The chances for local recurrence are great even after wide local excision. Squamous cell carcinomas and adenocarcinomas of the salivary glands are almost always highly malignant, quick to invade and metastasize, and fatal in a high percentage of the patients affected. This dense covering obscures the actual size of the underlying tumor, and the surgeon may be surprised by the size and extent of a lesion at operation.
